An example IRB application is available on the CNS server (in addition to the one available at the IRB site).
IRB Submission Process:
Once forms are complete, they need to be routed through the department prior to being sent to IRB.
Process:
-Send to Dr. Potts for Review****
-Dr. Potts returns to you with suggestions/approval.
-The IRB form is then signed and sent to Dr. Mullins (department head) for approval/signature.
-Dr. Mullins will then routes the application to the IRB.
***You may send directly to Dr. Mullins if:
1) The IRB request is only for a continuation (Application has already been approved and you are only requesting and extension in time from the IRB)
2) The IRB request is only for changes in superficial aspects of a study, such as placing the study online in addition to face-to-face measurement in the lab, or proposing to add additional participants.
*All new applications must be routed internally through the normal process*
